Your initial exam will be scheduled for approximately one hour. A diagnosis of your current oral condition will be made based on the combined findings from X-rays, a visual examination of all your teeth and verbal input from you describing any symptoms or concerns you may be experiencing. Oral hygiene instruction will be given with suggestions to help you care for your own teeth. In most cases, we will also clean your teeth on this visit. A written treatment plan will be given to you outlining all existing dental problems and proposed treatment.
Because we know your time is important, we try to stay on schedule and minimize waiting. Prior to your appointment, you will be mailed health history and registration forms, if time allows. These forms can also be downloaded from our website below.
Please assist us at the time of your first visit by providing the following information:
- Your health history and registration form
- Any X-rays from your previous dentist (if available)
- A list of medications you are currently taking
- Your dental and medical insurance card
You can download and print out these forms to bring on your first visit.
If you pre-medicate with antibiotics for dental appointments due to a heart murmur, artificial heart valve or joint replacement, please do so for this first appointment. If you need a new prescription, please let us know this when scheduling your appointment.
